Stellar Assist offers both face to face and virtual services for client's who are Self or Plan-Managed.
Support Coordination
Stellar Assist's Support Coordinators help NDIS participants understand, implement, and organise their plans. They assist in finding the right quality service providers based on individual needs and goals.
Specialist Support Coordination
Stellar Assist's Specialist Support Coordinators provide a higher level of support coordination for NDIS participants whose situations are more complex. They can assist with challenges in various support environments such as education, justice, health and housing.
Psychosocial Recovery Coaching
Stellar Assist's Psychosocial Recovery Coaches provide NDIS participants living with a psychosocial disability to live a meaningful and contributing life. They provide an evidence-based approach based on empowerment, measurable outcomes and capacity building underpinned by recovery-oriented principles.
